  • Social Insurance Numbers, full names, and dates of birth for you and all family members.

  • Last year’s Notice of Assessment.
  • Canada Revenue Agency correspondence, reassessments, or unfiled prior-year returns.

  • Employment income (T4).
  • Other income or payments (T4A).
  • Employment Insurance benefits (T4E).
  • Social assistance, disability benefits, or workers’ compensation (T5007).
  • Canada Pension Plan benefits (T4A(P)).
  • Old Age Security benefits (T4A(OAS)).
  • Other pension income.
  • RRSP or RRIF withdrawals (T4RSP, T4RIF).
  • Contract or construction-related payments (T5018).
